Why Do Indians Have Different BMI Thresholds?

The Research Evidence

Multiple large-scale studies involving Indian and South Asian populations have found that metabolic complications typically associated with obesity — including insulin resistance, type 2 diabetes, hypertension, and cardiovascular disease — begin to appear at significantly lower BMI values than in European populations.

Research published in The Lancet found that South Asians showed significant cardiovascular risk factors at a BMI of just 23–24, a level considered completely healthy by Western standards. At BMI 27.5, South Asians showed risks equivalent to those seen in Caucasians at BMI 30.

The WHO Expert Consultation on Appropriate BMI for Asian Populations (2004) recommended retaining the global cutoff points while recommending additional public health action points at 23 and 27.5 for Asian populations.

Visceral Fat & South Asians

The key mechanism is visceral fat accumulation. South Asians tend to store a higher proportion of their body fat as visceral (intra-abdominal) fat — the metabolically active fat that surrounds internal organs — compared to people of European descent at the same BMI.

Visceral fat is directly linked to insulin resistance, inflammation, and elevated cardiovascular risk. This means an Indian person at BMI 24 may have substantially more metabolically dangerous visceral fat than a person of European descent at the same BMI.
